We assist with securing initial enrollment and maintaining ongoing annual enrollment, ensuring participants continue to receive essential services. Whether transitioning back to a home of your choice after short- or long-term nursing home placement, or securing the waiver and non-waiver supports needed to prevent future nursing home stays, we are here to help. This includes services like home care, living skills training, behavioral support, medication management, nursing visits, and rehabilitation.
Our team connects you with both waiver and non-waiver providers in the community to ensure your health, safety, and well-being are maintained. We also help with maintaining critical benefits and entitlements, such as Medicaid, Medicare, Social Security, and SNAP.
By identifying your personal goals, we work with you to increase your independence and ensure you remain in your home, avoiding nursing home placement both now and in the future.
