At A1 Service Coordination, we provide personalized support to help individuals and families navigate services, access resources, and remain safely and successfully living in the community. We take the time to understand each person’s unique needs, goals, strengths, and challenges so we can build the right supports and connections together.
Our team assists with coordinating Medicaid Waiver services, connecting individuals to community resources, advocating during difficult situations, helping navigate Medicaid and healthcare systems, and supporting participants through changes and transitions over time. Whether you are newly exploring services or need ongoing support, we are here to guide you every step of the way with compassion, communication, and a person-centered approach.

Support for individuals with physical disabilities and seniors who require a nursing home level of care but prefer to live in their homes and communities. Our team takes a hands-on, personalized approach—coordinating services, connecting resources, advocating for participant needs, and helping individuals safely maintain their independence in the community.
We work closely with participants, families, providers, and healthcare professionals to build individualized service plans that reflect each person’s goals, strengths, and needs. From housing and community supports to waiver services and ongoing guidance, we are here every step of the way.

Support for individuals with traumatic brain injuries, with a focus on rebuilding independence, increasing community involvement, and supporting long-term success in the community. Our team takes a personalized, hands-on approach—coordinating services, developing individualized goals, connecting resources, and providing ongoing guidance every step of the way.
We understand that every brain injury and recovery journey is different. Our Service Coordinators work closely with participants, families, providers, and healthcare professionals to build meaningful supports that promote safety, stability, independence, and quality of life.

Advocacy is an important part of who we are at A1 Service Coordination. Beyond direct services, our team remains actively involved in legislative efforts, professional education, industry conferences, and statewide discussions focused on improving community-based supports and services.
We collaborate with organizations including the Brain Injury Association of New York State (BIANYS), the Brain Injury Association of America (BIAA), the Alliance of TBI & NHTD Waiver Providers, and the New York State Department of Health to help support meaningful change and strengthen services for the individuals and families we serve.
Our advocacy efforts also extend to issues impacting aging populations, dementia care, disability rights, community inclusion, and access to long-term supports and services.

A1 Service Coordination continues to grow and expand in order to better support individuals and families throughout the community. As needs evolve, we remain committed to developing additional programs, services, and supports that help individuals live safely, independently, and successfully in their homes and communities.
