Nursing Home Transition and Diversion (NHTD) Waiver Service Coordination
Support for individuals with physical disabilities and seniors who require a nursing home level of care but prefer to live safely in their homes and communities. Our team takes a personalized, hands-on approach—coordinating waiver and non-waiver services, developing person-centered service plans, connecting participants with trusted providers and community resources, and providing ongoing support to help maintain independence and quality of life.
We remain actively involved by monitoring services, addressing concerns, advocating for participant needs, and adjusting supports as needs change over time to help ensure each participant continues to receive the appropriate level of care and support.
Who This Program Is For
This program is designed for individuals who:
- Are age 18 or older
- Have a physical disability or are age 65+
- Are eligible for Medicaid
- Require a nursing home level of care
- Want to remain living in their home or community with the right supports in place
Enrollment in the NHTD Waiver is completed through the Regional Resource Development Center (RRDC), where individuals select an approved Service Coordination agency. A1 Service Coordination is an approved provider.

A1 Service Coordination helps coordinate and connect individuals to available services and providers based on their individual needs, goals, availability, and eligibility.

Please note: Not all services are available in every area or appropriate for every participant. A1 Service Coordination currently provides Service Coordination services and helps connect participants to other approved waiver and community-based providers.

Our team works closely with participants, families, discharge planners, providers, housing resources, and community supports to help coordinate the services needed for a successful transition home.
We understand that leaving a nursing home can feel overwhelming, which is why we take a hands-on approach throughout the process—helping address barriers, coordinate supports, connect resources, and develop individualized service plans focused on safety, independence, and long-term community living.

Support for individuals with traumatic brain injuries who require a nursing home level of care but prefer to live safely in their homes and communities. Our team takes a personalized, hands-on approach—coordinating waiver and non-waiver services, developing person-centered service plans, connecting participants with trusted providers and community resources, and providing ongoing support to help promote independence, stability, and quality of life.
We remain actively involved by monitoring services, addressing concerns, advocating for participant needs, and adjusting supports as needs change over time to help ensure each participant continues to receive the appropriate level of care and support while successfully living in the community.

• Have a diagnosed traumatic brain injury (TBI)
