A Service Coordinator helps individuals enrolled in the NHTD or TBI Waiver Program access, coordinate, and maintain the services and supports needed to live safely and independently in the community.
Service Coordinators work closely with participants, families, providers, and community supports to develop person-centered service plans, coordinate waiver and non-waiver services, monitor ongoing needs, assist with problem-solving, advocate for appropriate supports, and help connect individuals to community resources and services.
The goal of Service Coordination is to help support health, safety, independence, and community integration.
No. A Service Coordinator does not replace family members, caregivers, friends, or other informal supports. Service Coordination is designed to help coordinate services, advocate for needs, connect individuals to resources, and support person-centered planning within the waiver program.
Natural and informal supports often continue to play an important role in an individual’s life, care, decision-making, and overall support system.
Participants enrolled in the NHTD or TBI Waiver Program will have ongoing contact with their Service Coordinator, including at least monthly required communication and regular monitoring of services and supports.
The frequency and type of communication may vary based on the participant’s needs, preferences, current situation, and service plan. Contact may occur in person, virtually, by phone, or through other approved methods.
Response times may vary depending on the nature of the concern, staff availability, weekends, holidays, and whether additional coordination with providers or outside agencies is needed. In general, participants can expect communication and follow-up within 24–48 business hours for non-emergency matters.

o. Service Coordinators do not manage home care aide schedules, staffing, call-outs, shift coverage, or daily operations of home care agencies. Those responsibilities are handled directly by the licensed home care agency providing services.
However, Service Coordinators can help participants communicate concerns, coordinate with providers, address service-related issues, and assist with identifying alternative resources or providers when appropriate.
No. Service Coordinators do not make personal, medical, financial, or legal decisions for participants.

Service Coordinators assist with coordinating waiver and non-waiver services, connecting individuals to resources, advocating for needs, and supporting person-centered planning related to health, safety, independence, and community living.
However, Service Coordinators cannot perform tasks outside of their professional role or responsibilities, including providing direct care, managing finances, acting as legal representatives, completing personal errands, providing transportation, or performing duties that are the responsibility of other providers, agencies, family members, or supports.
Response times may vary depending on staff availability, the nature of the concern, business hours, holidays, and whether coordination with outside providers or agencies is required. The planning process is designed to promote independence, dignity, self-direction, and community inclusion.
Active Medicaid coverage is required for participation in the NHTD and TBI Waiver Programs and for many related services and supports. If Medicaid becomes inactive, suspended, or discontinued, waiver services may be impacted or unable to continue until coverage is restored.
A1 Service Coordination can help provide guidance, referrals, and coordination related to Medicaid renewal concerns and may assist individuals in connecting with appropriate resources or agencies involved in the Medicaid process.
Yes. A1 Service Coordination takes privacy and confidentiality seriously and follows HIPAA privacy and security guidelines regarding protected health information and personal information.
Information is only shared with authorized individuals, providers, agencies, or supports as permitted by law and with appropriate consent or authorization when required.
That is okay. Many individuals and families are unsure about what services, programs, or supports may be available when they first begin the process.
A1 Service Coordination can help provide information, discuss general needs and goals, explain available resources and waiver services, and help guide individuals through the process of identifying appropriate supports and next steps.
Enrollment timelines vary depending on the individual’s situation, required documentation, provider availability, Medicaid eligibility, assessments, housing status, and approval processes through outside agencies and the RRDC.
While some cases may move more quickly, others can take several weeks or months depending on the complexity of the situation and any delays related to medical records, Medicaid, provider selection, housing, or required approvals.
A1 Service Coordination understands that many individuals and families are navigating urgent and stressful situations. While A1SC works to provide timely communication, coordination, follow-up, and advocacy, we cannot guarantee expedited timelines for services, approvals, provider availability, Medicaid, housing, transportation, equipment, or other processes managed by outside agencies or organizations.
Timelines may vary depending on the individual situation, required documentation, provider capacity, program requirements, and outside agency processes.
